Job Summary
Participate as a member of the Interdisciplinary Team in the planning, implementation, and evaluation of care plans for frail elders under the PACE model. Conduct initial and ongoing assessments of health and nursing needs, then implement, develop, and document participant plans of care. Provide clinical direction to the team, physicians, and families while ensuring continuity of quality care through appropriate documentation. Perform on-call responsibilities to provide service to participants and families as assigned. Maintain positive interpersonal relations with employees, managers, and other department staff to maximize productivity.
Required Qualifications
- Associates Nursing Degree
- Registered Nurse License
- Minimum of one year's experience working with a frail or elderly population
- Compliance with any federal or State requirements for direct patient care staff in their respective settings
- Valid driver's license
- Current CPR Certification
- Pass a criminal background check
- Medically cleared of communicable diseases and up to date with immunizations
- Ability to be able to quickly recognize situation/problems before they become acute, and to be able to implement effective solutions and understand consequences
