Job Summary
Seeking a Registered Nurse Case Manager with a Bachelor's degree and 5 years of RN experience to participate in all phases of the Case Management Program. Responsibilities include developing treatment plans, collaborating with multidisciplinary teams, and ensuring continuity of care. Full-time position with no weekend or holiday work.
Required Qualifications
- Bachelor’s degree in Nursing or Master’s Degree in Nurse Case Management
- At least 5 years of RNCM experience within the past 7 years
- Current certification from accredited organization (e.g., CCM, CDMS, etc.)
- Current BLS Healthcare Provider certification
Desired Qualifications
- Ability to integrate case management with social work
- Proficiency in developing patient treatment plans
- Strong communication and collaboration skills
- Experience with automated programs and information technology in case management
- Knowledge of JCAHO, URAC, CMSA standards
