Job Summary
Provide case management, care coordination, and discharge planning for inpatient and outpatient settings at 56th Medical Group. Assess patients and families from diverse sources including discharge planning, the Medical Evaluation Board process, and medical records to evaluate physical, emotional, and social status. Develop patient-specific plans with short and long-term goals, collaborate with providers and team members for approval, and guide patients through healthcare systems to maximize resource use. Execute care plans, advocate for patients, ensure referrals are ordered, and monitor treatment adherence and outcomes. Document updates in electronic medical records, participate in interdisciplinary meetings, and maintain liaison with community agencies. Maintain productivity consistent with DoD, AF, and facility guidelines while completing required training and security clearances.
Required Qualifications
- Registered Nurse
- Minimum ASN, BSN preferred for Case Management
- graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), the Commission on Collegiate Nursing Education (CCNE)
- Three years of experience within the last four years as a registered nurse
- Current, full, active, and unrestricted license to practice as a Registered Nurse, any state
- Certified Case Manager by Commission for Case Management (CCM)
- Certification of Disability Management Specialists Commission: Certified Disability Management Specialist (CDMS)
- Association of Rehabilitation Nurses: Certified Rehabilitation Registered Nurse (CRRN)
- American Board for Occupational Health Nurses Certified Occupational Health Nurse (COHN)
- Certified Occupational Health Nurse-Specialist (COHN-S)
- National Board for Certification in Continuity of Care: Advanced Certification in Continuity of Care(ACCC)
- Commission on Rehabilitation Counselor Certification: Certified Rehabilitation Counselor (CRC)
- American Nurses Credentialing Center Nurse Case Manager (RN-NCM)
- National Academy of Certified Care Managers: Care Manager Certified (CMC)
- Completion of an accredited residency approved by the Accreditation Council for Graduate MedicalEducation (ACGME)
- current AHA or ARC BLS Healthcare Provider certification
- ability to pass a Government background check/security clearance
- knowledge and skills to effectively apply the following core case management functions: a) Assessment: Identification of patients for case management; comprehensive collection of patient information and medical status; and continued evaluation of an established plan of care
- knowledge and skills to effectively apply the following core case management functions: b) Planning: Collaboration with the patient, family/caregiver, primary provider and other members of the health care team for developing an effective plan of care
- knowledge and skills to effectively apply the following core case management functions: c) Facilitation: Care coordination and communication among all involved parties
- knowledge and skills to effectively apply the following core case management functions: d) Advocacy: Support for the patient and family/caregivers to ensure identified education and appropriate, timely care is received
- knowledge in medical privacy and confidentiality (Health Insurance Portability and Accountability Act [HIPAA])
- knowledge in accreditation standards of Accreditation Association for Ambulatory Health Care (AAAHC) and The Joint Commission (TJC)
- computer applications/software to include Microsoft Office programs, MS Outlook (e-mail), and internet familiarity
- skill and tact in communicating with people who may be physically or mentally ill, uncooperative, fearful, emotionally distraught, and occasionally dangerous
- organization, problem-solving and communication skills to articulate medical requirements to patients, families/care givers, medical and non-medical staff in a professional and courteous way
- Monday - Friday, 9 hours between 7:00am and 4:30pm, with a one hour lunch
- No weekends or holidays
