Job Summary
Lead the interdisciplinary care strategy from admission to discharge to ensure every patient experiences a seamless, safe, and well-coordinated transition home or to their next level of care. Advocate for patient needs and personal values by removing financial or logistical barriers, ensuring patients receive necessary resources and support for a successful recovery. Evaluate medical necessity and care levels to ensure patients receive the right treatment at the right time while optimizing hospital and insurance resources. Collaborate with families, clinical teams, and insurance providers to build a unified plan that prioritizes high quality outcomes and prevents gaps in post-discharge care.
Required Qualifications
- Registered Nurse credentialed from the Indiana Board of Nursing prior to hire or transfer
- BLS Provider preferred (American Heart Association or American Red Cross accepted)
- Diploma from an accredited nursing program OR professional licensure at time of hire
