Manage patient plans of care by performing comprehensive assessments, collaborating with multidisciplinary teams to develop and modify individualized care plans, and ensuring proper discharge planning. Complete timely and accurate documentation including OASIS assessments, admissions, and care coordination. Communicate changes in care plans, new orders, and lab results promptly to maintain continuity and facilitate interdisciplinary communication through team meetings and case conferences.
