Registered Nurse - Care Transition Unit - RN

Coordinate nursing care, direct patient/family education, and manage transitions of care for assigned patients within the Mayo Clinic Nursing Professional Practice Model. Plan, implement, evaluate, and communicate all phases of nursing care while delegating tasks based on skill level, experience, and patient acuity. Provide leadership through preceptor roles, informal and formal activities, and quality improvement efforts. Maintain excellent communication skills for mentoring and teaching, and participate in committees or projects as needed. A subset of employees may be required to drive their personal vehicle. This role is eligible for TN sponsorship and requires state licensure and VisaScreen certification.
