Job Summary
RN in Care Transition Unit responsible for coordinating nursing care, including direct patient care and patient/family education, with emphasis on transitions of care across the continuum. Leads and participates in quality improvement, mentoring, and informal leadership activities; may participate on committees/projects and guide others through the nursing process. May be required to drive a personal vehicle. Eligible for TN sponsorship; sponsorship requires state licensure and VisaScreen or equivalent certification.
