Job Summary
Registered Nurse Care Manager (RN) for Diabetes Clinic coordinates and manages patient care across settings. Design, implement, and evaluate treatment plans in collaboration with the healthcare team; empower patients to navigate their health journeys; provide care for Health Home enrollees and track outcomes; ensure evidence-based, high-quality care; requires BSN and Missouri RN license, BCLS, and at least three years of healthcare experience. Strong communication and team collaboration skills, with proficiency in Microsoft Office. Hours: 8:00 AM–4:30 PM, Full-time at University Health 4 (UH4) in Kansas City, MO.
Required Qualifications
- BSN (or equivalent combination of ADN and relevant experience)
- Current Missouri RN license
- Current BCLS (Basic Life Support) certification
- Minimum of three years working in a healthcare setting
- Ability to read and speak English fluently
- Proficient in Microsoft Word, Excel, and PowerPoint
