Job Summary
Assess members focusing on health and safety needs to develop comprehensive Member Care Plans, then coordinate with acute and primary care providers. Conduct regular health assessments, educate members on disease processes, nutrition, and lifestyle modifications, and document findings within required DHS contract timeframes. Participate in monthly phone contacts and travel approximately 40% of the time to visit members in their settings. Complete record reviews for reassigned members, identify remediation needs, and assign tasks in the D365 system while informing managers of significant risks. Work remotely from home with a valid driver's license and work Monday through Friday, 8:00 am – 4:30 pm CST.
Required Qualifications
- Associate degree in nursing
- Licensed Registered Nurse in the state of Wisconsin (in good standing)
- Valid state driver's license
- Proof of personal vehicle liability insurance with at least 100/300/100 limits
- TB screening eligibility (required to be screened for TB)
Desired Qualifications
- 1 year Care Management experience
- Reside within 45 mins or less of the assigned coverage area
- Previous Care Management experience
