Job Summary
Registered Nurse Care Manager – Home Care in Woodbridge, NJ. In this Per Diem role, the RN Care Manager designs person-centered care plans, mentors caregivers, assesses clients in home settings, and ensures compliance and quality. Responsibilities include completing in-home RN assessments, crafting individualized Care Plans with client/family approval, orienting caregivers to Care Plans, conducting field competency checks, monitoring vitals and condition changes in a HIPAA-secure EMR, coordinating with physicians and therapists, maintaining audit-ready client files, and upholding state home-care regulations. Requirements include an active NJ RN license, a Bachelor’s degree or 2+ years geriatric/home-care experience, CPR/First Aid certification, and a valid driver’s license. Required Qualifications
- Active Registered Nurse (RN) license in NJ
- Bachelor’s in Nursing or related human-services field – OR – 2+ years’ geriatric/home-care experience
