Job Summary
RN Care Manager for in-home care across Washington DC and Maryland. Responsibilities include in-home RN assessments, developing person-centered care plans with clients and families, orienting and supervising caregivers to Care Plans, monitoring client vitals and status, documenting in HIPAA-compliant EMR, escalating concerns promptly, and collaborating with Client Care Managers and Scheduling to ensure seamless transitions. Must hold an active Maryland and/or DC RN license, have a BSN or related healthcare degree (or 2+ years home care/geriatric nursing experience), CPR/First Aid, and a valid driver’s license. Role offers flexible scheduling, autonomy with clinical backup, mileage reimbursement, and benefits. Locality: Leonardtown, MD; Remote option: IN_PERSON for in-home care visits; candidate must reside in Maryland.
Required Qualifications
- Active Registered Nurse (RN) license in Maryland and/or Washington, DC
- BSN or related healthcare degree — or 2+ years of home care / geriatric nursing experience
- CPR/First Aid certification
- Valid driver’s license and reliable transportation
- Professional, compassionate communication with clients, families, and field staff
