Job Summary
Assess members focusing on health and safety needs to develop a comprehensive Member Care Plan. Conduct regular health assessments, coordinate with acute and primary care providers, and educate members on disease processes, nutrition, and lifestyle modifications. Document in member files within required DHS contract timeframes and participate in monthly phone contacts while traveling to attend in-person visits in members' settings. Work remotely from home with approximately 40% travel time to the assigned coverage area in Taylor County, WI. Maintain a valid driver's license and personal vehicle insurance for work-related travel, with mileage reimbursement provided.
Required Qualifications
- Associate degree in nursing
- Licensed Registered Nurse in the state of Wisconsin (in good standing)
- Valid state driver's license
- Proof of personal vehicle liability insurance with at least 100/300/100 limits
- TB screening
Desired Qualifications
- 1 year Care Management experience
- Reside within 45 mins or less of the assigned coverage area
- Previous Care Management experience
