Job Summary
The Registered Nurse - Care Management plans, organizes, and provides nursing services to clinic patients in conjunction with clinic providers, focusing on individuals with chronic disease conditions and/or complex needs. The role involves educating patients and families, facilitating communication, coordinating services, addressing barriers, and promoting optimal allocation of resources while balancing clinical quality and cost management. The position supports collaboration with the health care team to improve patient health outcomes and requires completing education in nursing (BSN or ADN) and maintaining licensure in Washington State. The role includes a sign-on bonus potential and a competitive Seattle/Puget Sound salary in a part-time schedule (minimum 24 hours/week).
Required Qualifications
- Education: Completion of a BSN, or an ADN from an accredited institution.
- Experience: Two years ambulatory nursing experience with BSN or three years with an ADN preferred.
- Current WA State Registered Nurse (RN) license.
- Basic Life Support (BLS) for Healthcare Providers.
- Obtaining specialty certification in care management within 12 months of hire required.
