Registered Nurse Care Coordinator – Chronic Disease

Job Summary
Deliver comprehensive chronic disease management, care coordination, and care navigation in community settings while conducting holistic health assessments and developing individualized care plans. Support goal setting and self-management through health education, coordinate referrals to multidisciplinary services including social prescribing, and provide post-discharge follow-up with escalation for clinical deterioration. Collaborate with GPs and multidisciplinary teams to support integrated care models, participate in case conferencing and clinical huddles, and maintain accurate clinical documentation. Work autonomously across the South Western Sydney catchment with a preference for Wednesday, Thursday, and Friday shifts, reporting to Andrea McDougall.
Required Qualifications
- Bachelor of Applied Science (Nursing) or equivalent
- Division 1 Registered Nurse registration with AHPRA
- Minimum Grade 2 Year 4 Registered Nurse experience
- 0.6 FTE
- Max Term until June 2027
- Flexible with days, Wednesday, Thursday, Friday preferred
- Based in Campbelltown on the lands of the Dharawal People
- community-based outreach work across the South Western Sydney catchment required
- Experience in chronic disease management and supporting people with complex needs
- Strong assessment, care planning and care coordination skills
- A client-centred approach grounded in cultural safety and respect
- Confidence engaging with GPs, practice staff and community partners
- Strong communication, organisation and time‐management skills
- Ability to work independently across community settings and as part of a team
Desired Qualifications
- Experience in primary or community health settings
- Understanding of the social model of health
- Experience working with culturally and linguistically diverse communities
- Confidence using digital health systems and clinical software
