Collaborate in establishing patient care plans, coordinate implementation of care delivery, and anticipate patient needs across the continuum. Analyze comprehensive information to determine nursing diagnoses, evaluate patient progress toward goals, and apply appropriate interventions to ensure effective outcomes. Conduct primary care patient interviews and physical examinations while providing patient education in practical applications of moderate difficulty. Search for evidence to answer clinical questions and contact the primary care provider or healthcare team to resolve problems. Work Monday through Friday from 8 a.m. to 4:30 p.m., with no nights, weekends, or holidays.
