Job Summary
Assess, plan, implement, and evaluate individualized resident care plans while administering medications and treatments accurately. Closely monitor residents' conditions, communicate significant changes to physicians, and provide leadership to LPNs and CNAs. Maintain thorough documentation in compliance with regulations and partner with interdisciplinary teams to deliver coordinated care. Respond swiftly to emergencies with calm interventions and educate residents and families on health conditions and wellness strategies. Work 2nd shift with an active PA RN license and CPR/BLS certification. Join a supportive, resident-centered team at Carbondale Rehabilitation and Healthcare Center focused on geriatric care.
Required Qualifications
- Active and unrestricted RN license in the state of Pennsylvania (PA)
- Current CPR/BLS certification
