Registered Nurse - Brooklyn (Lenox Rd.)

Assess persons supported medical conditions to develop targeted care plans that prevent illness and manage chronic conditions. Conduct regular health assessments, administer medications and treatments, and monitor for adverse reactions. Coordinate with physicians, families, and healthcare providers to align care efforts and reduce avoidable hospitalizations. Provide telephone triage and consultation to identify urgent health concerns. Train and oversee unlicensed staff in medication administration, infection control, and emergency protocols. Ensure accurate documentation and maintain compliance with state and federal regulations. Requires current New York State RN license, 1+ years of experience, and BLS certification.
