Job Summary
Coordinate and deliver health care services for assigned residences within the residential program, conducting evaluations, diagnoses, and therapeutic interventions per regulatory standards. Develop, supervise implementation of, and revise written health care plans for each resident while monitoring progress and integrating therapeutic techniques. Administer medications, conduct vital signs checks, and provide immediate first aid with referrals when necessary. Train staff on medical administration, infection control, and adaptive equipment, and handle communicable disease protocols through assessment and reporting. Participate in interdisciplinary team meetings, annual reviews, and medical reviews to ensure comprehensive care for individuals with developmental disabilities. Maintain accurate records of examinations, immunizations, and incidents, and carry the agency cell phone for on-call response regarding treatment plans.
Required Qualifications
- License and current registration as a Registered Nurse in the State of New York
- CPR Certification
- Ability to read, write, speak and understand English
- Good oral and written communication skills
- Good organizational skills
- Interpersonal skills necessary to interact effectively with coworkers, employees, individuals, families and outside contacts
- Must be able to maintain the strictest confidentiality
- Knowledge of equipment typically used by persons receiving services, including wheelchairs and adaptive devices
- Must be willing to work at various program sites
- Associate's Degree (if completed before 2017) -or- Bachelor's Degree Required
- At least one year of experience as a Registered Nurse in the delivery of community-based health services to individuals with developmental disabilities
