Job Summary
Registered Nurse Home Health role at Elara Caring involves caseload assessment, planning and coordination of patient care in home settings, ongoing evaluation of nursing needs, travel within service area (up to 50%), collaboration with interdisciplinary team, and ensuring HIPAA-compliant care documentation. Requires an active RN license, 1+ year clinical experience, ability to travel, and willingness to lift/move patients and equipment; associates (or higher) degree in nursing is preferred. Required Qualifications
- Current, unrestricted state RN license
- 1+ year experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws
- Associates Degree or Bachelor’s Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
- Previous experience with HomeCare HomeBase is preferred
