Registered Nurse- Bilingual

Support patient assessment and prioritization for walk-in and telephonic triage to ensure patients receive the right level of care and avoid unnecessary hospital visits. Execute protocol-driven chronic condition management, perform skilled nursing procedures including wound care and medication administration, and conduct onsite testing such as point-of-care labs and EKGs. Provide comprehensive patient and caregiver education regarding chronic conditions and preventative care, while reviewing high-priority abnormal laboratory results to maintain patient safety. Partner with providers and the interdisciplinary team to manage complex care plans and coordinate weekly transitions for patients moving from the hospital back to the clinic. Work full-time, 40 hours weekly, with a base salary range of $54,095.00 - $116,760.00. Requires active, unrestricted RN licensure, BLS certification, and fluent English skills; fluency in Spanish, Polish, Russian, or other community languages is strongly preferred.
