Job Summary
Perform nursing assessments to identify risk factors for individuals with severe and persistent mental illness, including evaluating blood pressure, weight, and BMI. Act as a liaison between public and private healthcare facilities to carry out referrals and incorporate measurable, disease-specific goals into treatment plans. Make home visits to administer services or assess conditions in the individual's environment, and accompany clients to medically complex appointments. Provide crisis intervention, supportive interaction, and education to foster self-management skills while participating in interdisciplinary treatment team meetings. Assist the Nursing Supervisor in maintaining clinical staffing within the medication clinic, including administering IM medications and Mantoux testing. This full-time role (40 hours) supports Community Support Services' mission to provide quality behavioral health care in Summit County, Ohio.
Required Qualifications
- Graduate of accredited nursing program
- Valid license to practice as an RN in the State of Ohio
- Experience providing nursing services to persons with severe and persistent mental illness in a community and clinic setting
- A car
- Valid Ohio Driver's License
- Insurability through the agency's insurance carrier
- Knowledge of nursing rules and regulations in the State of Ohio
- Discernment regarding scope of practice issues
- Thorough knowledge of nursing standards and practice
- Excellent organizational skills
- Attention to detail
- Communication skills
- Ability to work in a team environment
