Job Summary
Oversee care for individuals with intellectual and developmental disabilities by traveling to homes for medication management, medical treatment, and education of Direct Service Providers. Conduct daily follow-ups, triage medical concerns, and perform assessments including diabetic foot checks and vital signs monitoring. Coordinate complex care plans, schedule appointments, and educate staff on medical equipment use while auditing medication rooms and EMAR systems. Participate in on-call rotations and assist with policy development and controlled substance destruction. Ensure compliance with state requirements and cooperate during inspections.
Required Qualifications
- Valid RN license
- Must pass criminal background check
- Must possess a valid Driver's License
- Proficient in speaking, reading and writing the English language
- Approved by state, federal and government entities to work within BSLS programs
- Ability to use office equipment, including BSLS information technology systems
- General knowledge of nursing and healthcare terminology
- Proven capability of communicating with multiple disciplines
- Proven ability to deescalate difficult situations
- Proven ability to be able to provide care as needed
- Excellent computer skills
- Excellent communication skills
- Proficient in speaking, reading and writing the English language required
Desired Qualifications
- minimum of two (2) years previous experience preferred
