Job Summary
Deliver medication management, health assessments, and wellness education in community settings while providing crisis intervention and participating in on-call rotation. Coordinate care with hospitals, primary care providers, and community partners to support clients in developing self-management skills and achieving personal recovery goals. Maintain accurate documentation and participate in regular team meetings and case reviews as part of a multidisciplinary Assertive Community Treatment team. Licensed Registered Nurse (RN) in Maryland with experience in community mental health is required, along with a valid driver's license.
