Job Summary
Conduct home health visits to clients in designated geographic territories, administering medications, performing wound care, and executing treatments per designated care plans. Monitor client conditions and report changes to Clinical or Client Services Managers while accurately documenting observations, interventions, and evaluations using a touch pad tablet. Manage case coordination and follow up on doctors' orders as part of a multi-disciplinary team providing skilled nursing and rehabilitative care. Requires a current SC RN license, one year of recent clinical experience, and ability to travel to assigned cases.
Required Qualifications
- A current license as a Registered Nurse in SC
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Graduation from an accredited and approved nursing program
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned
Desired Qualifications
- Prior home care experience
- Prior experience with electronic medical records (EMR)
