Job Summary
Provide nursing services, treatments, and diagnostic procedures for patients in the Inpatient Unit, Care 24 field, and Continuous Home Care. Assess patient status, report changes to physicians, and coordinate care with interdisciplinary team members and community resources. Assist in developing and revising care plans, teach families on pain management and self-care techniques, and supervise care provided by LPNs and Home Health Aides. Document accurate assessments and interventions in the EMR at the point of care. Participate in clinical education and staff meetings while applying knowledge of state and federal regulations. Work variable shifts including weekend rotation and emergency situations as staffing needs require.
Required Qualifications
- Current Registered Nurse License to practice in the state of Florida
- Healthcare Provider CPR required upon hire and maintained
- Valid Florida Driver's License
- Associates Degree in Nursing
- Bachelor in Nursing (BSN) degree
- CHPN
- Hospice experience
- Communication skills of reading, writing, and speaking in English
- Math skills needed including addition, subtraction, multiplication, division and percentages/fractions with ability to calculate medication doses
- Must successfully pass the math test given at time of interview prior to starting orientation and annually
- Must be able to lift and/or move up to 10 pounds
- Must be able to occasionally lift and/or move up to 50 pounds
- Must be able to endure exposure to common household allergens (i.e., pets, plants, dust)
- Must be able to drive to patient homes
- Must be able to utilize device to document in Electronic Medical Record (EMR)
- Must be available for variable shifts, including weekend rotation
- Must follow the Comprehensive Emergency Management Grid during times of activated emergency situations
- Must be a Nicotine Free individual
