Job Summary

Conduct nursing services, treatments, and diagnostic procedures in the Inpatient Hospice House in Naples, FL, utilizing the nursing process to assess, plan, and manage complex end-of-life symptoms. Teach patients and families self-care techniques, pain management strategies, and medication instructions while supervising LPNs and Home Health Aides. Coordinate interdisciplinary team meetings and care plans, documenting accurate assessments and interventions in the EMR at the point of care. Cross-train across departments including Care 24 field, Triage, and Continuous Home Care as staffing needs require, adhering to state and federal hospice regulations.

Required Qualifications

  • Current Registered Nurse License to practice in the state of Florida
  • Healthcare Provider CPR required upon hire and maintained
  • Valid Florida Driver's License
  • Associates Degree in Nursing
  • Bachelor in Nursing (BSN) degree
  • CHPN
  • Hospice experience
  • Communication skills of reading, writing, and speaking in English
  • Math skills needed including addition, subtraction, multiplication, division and percentages/fractions with ability to calculate medication doses
  • Must successfully pass the math test given at time of interview prior to starting orientation and annually
  • Must be able to lift and/or move up to 10 pounds
  • Must be able to occasionally lift and/or move up to 50 pounds
  • Must be able to endure exposure to common household allergens (i.e., pets, plants, dust)
  • Must be able to endure exposure to outside weather conditions
  • Must be able to utilize device to document in Electronic Medical Record (EMR)
  • Must be able to drive to patient homes
  • Must be Nicotine Free
  • Must be available for weekend rotation per staffing needs
  • Must be available for departmental holiday rotation
  • Must be able to participate as needed in variable shifts
  • Must be able to follow the Comprehensive Emergency Management Grid during times of activated emergency situations
  • Must be able to apply working knowledge of state, federal local and accreditation regulations for the delivery of hospice services
  • Must be able to document accurate assessment of patient status and updates the plan of care as needed in the EMR
  • Must be able to meet productivity expectations as set by the organization
  • Must be able to supervise care provided by LPN and HHA's

Desired Qualifications

  • BSN degree preferred
  • CHPN preferred
  • Hospice experience preferred
  • Strong computer skills including database software and general knowledge of email/outlook
