Job Summary
MDS Coordinator responsibilities include conducting and coordinating resident assessments in accordance with regulations and facility protocols, monitoring residents for condition changes, communicating assessment updates to the interdisciplinary team, overseeing the completion of MDS, quarterlies, CAAs, and care plans with input from Social Services, Activities, and Dietary, developing and updating care plans based on assessments, maintaining and distributing the MDS schedule, and submitting MDS data to the state weekly as required. Requires active RN (preferred) or LPN license and at least one year of MDS coordination experience in long-term care. This is a part-time on-site role at a skilled nursing facility in Lancaster, PA, with a comprehensive benefits package and a flexible schedule.
Required Qualifications
- Active Registered Nurse (RN) preferred or Licensed Practical Nurse (LPN) license in this state
- Minimum of one year of MDS coordination experience in a long-term care setting
