Job Summary
Collaborate with the interdisciplinary team to develop and implement comprehensive care plans for residents. Conduct thorough assessments and evaluations to identify resident needs and preferences. Ensure compliance with federal, state, and local regulations, including MDS guidelines and regulations. This Registered Nurse Assessment Coordinator role supports individualized care plan creation at Champion City Nursing & Rehabilitation under new ownership. Full-time 8-hour shifts with a competitive salary range of $80,000 to $90,000 annually and a $7,500 sign-on bonus are available.
