Job Summary
Conduct comprehensive client assessments addressing physical, psychological, social, and cultural needs while developing and evaluating individualized Care Plans in partnership with clients, families, and health professionals. Perform specialty-specific procedures, monitor treatment responses, and adjust care plans as necessary. Communicate effectively with clients and families to provide education and support, ensuring accurate documentation of all care activities. Advocate for ethical decision-making and coordinate interdisciplinary care plans across the Perth Metro and Peel regions. Contribute to policy development, education initiatives, and research projects. This community-based nursing role supports people living with dementia and their carers, offering 0.6 FTE (3 days per week) with a competitive salary and salary packaging up to $18,550 tax-free.
Required Qualifications
- Sound knowledge of Aged Care Quality Standards (ACQSC)
- Knowledge of the Support at Home Program (formerly HCP) and CHSP
- Demonstrated experience supporting people living with dementia
- Strong community-based clinical experience
- Advanced communication and stakeholder engagement skills
- Well-developed assessment and care planning skills
- Ability to work autonomously and collaboratively as part of a team
