Job Summary
Registered Nurse role delivering client-centered care in community-based dementia services north of the river, based in Shenton Park WA. Key duties include comprehensive client assessments, developing and evaluating individualized care plans with clients, families and health professionals, performing specialty procedures, educating and supporting clients and families, ensuring accurate documentation, advocating for ethical decision-making, coordinating interdisciplinary care, contributing to policy development and education initiatives, and mentoring junior staff. Requires sound knowledge of ACQSC, CHSP/Support at Home Program, and experience supporting people living with dementia; strong communication and stakeholder engagement skills; ability to work autonomously and as part of a team.
