Registered Nurse - Advanced Care at Home - RN

Registered Nurse - Advanced Care at Home (ACH) role delivering hospital-at-home care through telemedicine. The ACH RN performs patient assessment, medication verification, patient education, patient acquisition, and discharge while collaborating with a multidisciplinary team of nurse practitioners, physician assistants, RNs, case managers and pharmacists. Works within the ACH Command Center to coordinate care remotely and also provides coverage on inpatient units as needed, using telemedicine to monitor and treat hospitalized patients in their home setting.
