Assess patient needs and develop individualized plans of care for acute care patients in hospital or inpatient settings. Administer direct patient care, including medication administration, treatments, and patient safety protocols. Collaborate with interdisciplinary teams to manage units and delegate tasks to healthcare staff appropriately. Document care consistently while advocating for patients and families. Provide compassionate care in hospital, virtual, and in-home settings, including traveling to patient homes for Hospital Care at Home duties. Maintain compliance with all policies and attend staff development meetings.
