Job Summary
Conduct initial admission processes for hospice patients, initiating care plans based on immediate needs and communicating with the interdisciplinary team. Perform comprehensive nursing assessments to identify physical, social, and emotional needs, analyze data for hospice appropriateness, and prepare accurate medical records for diagnosis and reimbursement compliance. Manage case management duties as needed while upholding program standards, perfect documentation, and HIPAA confidentiality guidelines. Collaborate with physicians and staff to lead program development and ensure adherence to federal, state, and local laws. Maintain professional boundaries, handle sensitive issues with sound judgment, and embrace a flexible schedule to meet patient and family requirements.
Required Qualifications
- Current Florida license as RN, or current licensure from another state or compact license
- Must be Certified Hospice and Palliative Registered Nurse (CHPN) within 12 months
- Valid driver's license and automobile insurance in the State of Florida
- Reliable transportation to meet visit schedules
- Minimum two (2) years RN experience where physical assessment skills were utilized
- Must be an RN graduate from a state approved school of nursing
- Current (BLS) CPR certification required
- Must be able to handle sensitive issues appropriately using sound judgment, maintain professional boundaries, and maintain confidentiality
- Must be able to effectively plan, organize and prioritize work
- Be self-directed and able to work independently in the field
- Must have the capacity to embrace a flexible work schedule to meet the patient and family needs
- Must provide initially and update as appropriate all required information for credentialing at area healthcare facilities and hospitals, and maintain good standing with each
- Must be able to perform emergency duty before, during and/or beyond normal work hours or days in the event of an emergency, crisis situation or disaster (man-made or natural) including evacuation sites
- Must adhere to all smoking restrictions
- Must comply with all Federal, State and Local laws as well as the organization's compliance program
- Must be able to perform each essential duty satisfactorily
Desired Qualifications
- Bachelor's degree in nursing preferred
- Knowledge of Hospice Admissions criteria preferred
- Prior experience in oncology, home health, Hospice or palliative care setting is preferred
