Job Summary
Review all orders for new admissions, transfers, and discharges while anticipating patient and family needs. Coordinate interdisciplinary discharge planning with case managers and social services to facilitate access to outpatient resources. Conduct patient follow-up at 3, 7, 14, and up to 30-day intervals as needed. Serve as a liaison between medical teams and families post-discharge to ensure safe transitions of care. Maintain a safe environment, comply with National Patient Safety Goals, and act as a mentor to direct reports. Float to other units in the role of ADT as necessary.
Required Qualifications
- Bachelor's degree
- Current/valid Florida nursing license from a nationally accredited school of nursing
- CPR/BLS
- 3 years of recent nursing experience
- Strong interpersonal communication skills
- Proven track record of building relationships with physicians, direct reports, and key staff
- Moderate to advanced computer literacy
- Maintains professional certifications/licensure in active status and good standing
- Practices within the scope and guidelines set forth by the Florida Board of Nursing
- Demonstrates dependability/reliability by promptly reporting to work when scheduled
- Float to other units in the role of ADT, as necessary
