Job Summary
Facilitate hospice referrals and complete initial patient assessments to determine care needs and criteria. Establish relationships with community referral sources and coordinate admission processes with intake departments, physicians, and skilled nursing facilities. Provide physical assessments, nursing diagnoses, and patient/family education while maintaining assigned case loads as a Case Manager. Participate in Quality Assurance Performance Improvement processes and Interdisciplinary Team meetings to develop patient plans of care. This role supports Bristol Hospice's mission of providing family-centered hospice services through comprehensive community education and care coordination.
Required Qualifications
- Must have a current RN license in the state
- Must have a minimum of one (1) year of experience as a professional nurse within the last three (3) years or have a bachelor's degree in nursing from a program accredited by the National League of Nursing
- Must be able to travel 20% of the time, often without notice
- Must be flexible in work hours
- Must possess the willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
Desired Qualifications
- An Understanding of hospice philosophy and comfortable providing specialized care to the terminally ill
- Ability to work autonomously with little direct supervision
- Successfully demonstrate tact, patience and good personal hygiene
- Ability to communicate effectively
- Strong organizational skills
- Understanding of plan of care documentation
- Flexible in work hours to ensure the patients receive care as provided in the plan of care
- Ability to empathize with the needs of the ill, injured, frail and impaired
- Comfortable addressing issues of death/dying
- Calm demeanor when entering the patients home
