Job Summary
Admissions RN for hospice services conducting the initial admission process, initiating the plan of care based on identified needs, and communicating with the interdisciplinary team (IDT); responsibilities include applying hospice admissions criteria, maintaining documentation to support COP/CMS guidelines, coordinating with clinical staff, and ensuring professional, confidential interactions with patients and families; requires active RN licensure in Florida (or compact), CHPN within 12 months, valid driver's license, reliable transportation, and a minimum of 2 years of RN experience with strong assessment, organizational, and communication skills; BSN preferred and ongoing credentialing support; OSHA category I exposure; ability to work flexible hours and perform essential duties with permissible accommodations.
Required Qualifications
- Current Florida license as an RN, or current licensure from another state or compact license
- Must be CHPN within 12 months
- Valid driver’s license and automobile insurance in the State of Florida
- Reliable transportation to meet visit schedules
- Minimum two (2) years RN experience where physical assessment skills were utilized
- Must be an RN graduate from a state approved school of nursing
- Bachelor’s degree in nursing preferred
- Knowledge of Hospice Admissions criteria preferred
- Current (BLS) CPR certification required
