Job Summary
Field-based Registered Nurse (RN) in McHenry, IL and surrounding areas delivering skilled nursing visits in patients' homes. Responsibilities include nursing assessments and planning, determining eligibility and admissions processing, coordinating with the interdisciplinary team and attending physician, updating care plans, performing therapeutic, preventative and rehabilitative nursing procedures, pronouncing death when applicable, and maintaining accurate patient records. Requires licensure in Illinois, CPR certification, and reliable transportation; 1+ year clinical experience (preferably ICU, rehab, or medical-surgical) and home health experience preferred.
Required Qualifications
- Graduate from an accredited registered nursing program
- Licensed in the state of Illinois
- CPR certification
- Possess and maintain reliable transportation, current driver's license and automobile insurance
