Job Summary
Home Health Registered Nurse (RN) needed in Shelby County, TN to perform patient assessments, admissions, ongoing care planning, and deliver therapeutic, preventive, and rehabilitative nursing care in patients’ homes. Coordinate care with physicians and interdisciplinary teams, monitor patient progress, maintain timely documentation, and support patients and families through all care stages. Requires active RN license, graduation from an accredited nursing program, minimum 1 year of clinical experience (ICU/Med-Surg/Rehab preferred), CPR certification, and reliable transportation with a valid driver’s license. Mileage reimbursement and weekday daytime schedule offered.
