Registered Nurse

RN to manage a home health caseload in Downers Grove, IL and surrounding areas. Responsibilities include performing the nursing process (assessment, eligibility, admissions processing, planning, implementation, evaluation), gathering data across physical, psychological, social, and spiritual factors, coordinating intervention with the interdisciplinary team, initiating communication with the attending physician and other agencies, conducting visits to assess needs and update the care plan, performing therapeutic, preventive and rehabilitative nursing procedures, pronouncing death as required by licensure, maintaining accurate patient records and care plans, and ensuring timely documentation. Requires valid IL RN license, CPR certification, and reliable transportation. Mileage reimbursement and comprehensive benefits offered.
