Job Summary
Provide skilled nursing care according to physician orders and patient care plans while conducting assessments and ongoing evaluations. Coordinate care by communicating patient status changes with physicians and the interdisciplinary team, and educate patients and families on disease management, medications, and home care needs. Complete accurate clinical documentation, including OASIS assessments, and participate in care planning, case conferences, and quality improvement activities. Supervise LPNs, CNAs, and Home Health Aides in accordance with regulatory requirements to support patients' preventive, rehabilitative, and ongoing healthcare needs. Serve patients in Denver, Broomfield, and surrounding Northern and Southwest Colorado communities.
Required Qualifications
- Active Registered Nurse (RN) license
- Graduate of an accredited nursing program
- Current CPR certification
- Strong clinical assessment, communication, and organizational skills
- Ability to work independently and manage time effectively
Desired Qualifications
- Minimum one (1) year of nursing experience
- Home health experience
