Registered Nurse - $5,000 Sign-On Bonus

Registered Nurse at Oak Street Health providing competent nursing care and coordinating care across the patient and support team. Responsibilities include responding to telephonic requests, triage and symptom management, medication management and prior authorizations using standardized protocols, reviewing and updating patient medications, patient education and direct care for chronic conditions (e.g., diabetes, CVP), monitoring care plan implementation across provider panels, delegating tasks to clinical team members, building relationships with patients and caregivers to support preventative care and ED/hospital diversion, creating a welcoming environment focused on equity of care, participating in care team meetings and daily huddles, delivering an exceptional patient experience through service and respectful care, performing point-of-care testing and possibly phlebotomy, and performing other related duties. Required qualifications emphasize active RN licensure, BLS, EMR experience, clinic-based patient care, HIPAA compliance, strong communication, independence, and teamwork.
