Job Summary
Registered Dietitian focused on end-of-life care in a home setting. Plans, organizes and conducts dietetic education and supports patients and caregivers as part of an interdisciplinary clinical care team. Develops and implements a nutritional plan of care based on comprehensive assessments, counsels individuals and families on dietary principles, and provides home follow-up care by visit or phone. Documents nutritional data in clinical records, participates in interdisciplinary meetings, and ensures confidentiality and safety of records while complying with organizational policies. Requires Wisconsin RD credential, Master’s degree in dietetics, and driving access for home-based care.
Required Qualifications
- Wisconsin Registered Dietitian Credential
- Master’s degree in dietician studies from an accredited institution
- Ability to pass background check
- Ability to pass drug test
- Valid State Driver’s License and access to an automobile in good working order
- Automobile insurance in accordance with state and organizational requirements
- Acceptable driving record
