Job Summary
Develop and implement nutritional plans of care based on comprehensive assessments of patient needs. Provide counseling to individuals and families in the home setting, instructing on nutritional principles, dietary plans, food selection, and economics. Reevaluate nutritional care through home follow-up visits or phone calls. Communicate dietary history and care data through written records, documenting assessments, treatment plans, and progress notes. Attend interdisciplinary group meetings and actively participate in quality assessment and performance improvement teams. Maintain confidentiality of patient records while complying with organizational policies. Requires a Wisconsin Registered Dietitian credential, Master's degree, and valid driver's license.
Required Qualifications
- Wisconsin Registered Dietitian Credential
- Master's degree in dietician studies from an accredited institution
- Ability to pass background, consumer report, and drug test
- Valid State Driver's License
- access to an automobile in good working order
- automobile insurance in accordance with state and organizational requirements
- an acceptable driving record
- Pushing/Pulling and Lifting/carrying up to 12 pounds
- Occasionally up to 20 pounds
- Ability to adapt to a variety of work conditions to ensure the highest quality of patient care in a home setting
