Job Summary
Oversee and provide optimal nutrition therapy to a case load of clients by developing care plans through nutrition assessments and monitoring medical conditions. Work directly with patients and families to provide support and nutritional education, while contributing to multidisciplinary case conceptualization and treatment planning. Deliver individualized nutrition therapy, conduct assessments per policy and regulatory standards, and provide group nutrition education as assigned. Support plating and active meal coaching for clients in Therapeutic Supported Meals, and manage nutrition-specific transition and aftercare planning with internal and external dietitians. Document all nutrition care clearly in the EHR. Full-time, 40 hours/week, Monday-Thursday 11am-7pm, Friday 8am-4pm.
Required Qualifications
- Current Registered Dietitian license in the state of employment
- Current board certification with the Commission on Dietetic Registration
- Master's Degree
- ServSafe Food Handler certification
- Additional food safety certification as required by the state or municipality
- Monday-Thursday 11am-7pm
- Friday 8a-4pm
