Job Summary
Develop and implement nutritional plans of care based on comprehensive assessments of patient needs as part of the total care plan. Provide counseling to individuals and families in the home setting by evaluating and instructing on nutritional principles, dietary plans, food selection, and economics. Reevaluate nutritional care through home follow-up visits or phone contact while communicating dietary history and care data through written record systems. Record subjective and objective information, treatment plans, and progress notes in clinical records, including discharge summaries. Attend interdisciplinary group meetings and actively participate in quality assessment and performance improvement teams. Maintain confidentiality and safety of patient records while complying with organizational policies and procedures.
Required Qualifications
- Wisconsin Registered Dietitian Credential
- Master's degree in dietician studies from an accredited institution
- Ability to pass background, consumer report, and drug test
- Valid State Driver's License
- access to an automobile in good working order
- automobile insurance in accordance with state and organizational requirements
- an acceptable driving record
- Pushing/Pulling and Lifting/carrying up to 12 pounds
- Occasionally up to 20 pounds
- Ability to adapt to a variety of work conditions to ensure the highest quality of patient care in a home setting
