Job Summary
Registered Dietitian needed to provide ministry-funded, community-based dietitian services in clients' homes and/or via virtual care in Oshawa, ON. Independent contractor with a hybrid caseload and optional virtual visits. Conduct dietitian assessments using interviews, standardized tools, and informal assessment techniques across geriatrics, oncology, chronic disease management, and palliative care. Develop and implement best practice interventions in collaboration with clients/caregivers, provide education to clients and caregivers, and connect them to community resources. Maintain accurate documentation including assessments, care plans, progress and discharge summaries aligned with standards. Require registration in good standing with the College of Dietitians of Ontario and a valid G2 or G driver’s license with access to a reliable vehicle for community travel. Strong organizational, verbal/written communication, and MS Office proficiency; ability to work independently while collaborating with an inter-professional team; new graduates welcome; flexibility to address overflow in other Durham Region areas as needed.

- Registered Dietitian in good standing with the College of Dietitians of Ontario
