Job Summary
Assess and monitor residents' nutritional status, provide recommendations to medical staff, and develop adequate care plans. Evaluate special nutrition needs including nutrition support, skin breakdown, and weight issues; coordinate procurement of nutrition supplies and oral supplements with vendors; monitor meal service for diet modifications; educate residents, families, and staff on nutrition concepts and diet modification; regularly evaluate and adjust dietary care plans with an interdisciplinary team; collaborate with Speech-Language Pathologist to ensure mechanically altered diets for dysphagia; review menu changes for policy, state, and federal guideline compliance; update diet orders and menus in EMR/menu software daily; supervise dietary team and conduct routine quality audits to ensure high-quality care; communicate with the interdisciplinary team daily; the facility is a 200-bed rehabilitation and skilled nursing facility located in White Plains, NY, and is part of the Centers Health Care Consortium; sign-on bonus of $5,000 and Daily Pay are offered.
Required Qualifications
- Must be a Registered Dietitian
- Degree in Nutrition or Dietetics
- Exceptional interpersonal & leadership skills
- Knowledge of applicable state and federal guidelines
- Computer literacy and proficiency with EMR software and computer-based menu systems
