Job Summary
Assess and monitor residents' nutritional status; develop and implement care plans; coordinate nutrition support with outside vendors and department heads; ensure diet modifications are followed in meal service; educate residents, families, and staff on nutrition concepts; review and adjust dietary care plans with the interdisciplinary team; collaborate with Speech-Language Pathologist for dysphagia-related diets; update diet orders and menu changes in EMR; supervise the dietary team; conduct quality audits and ensure compliance with regulations and policies; communicate daily with the interdisciplinary team to provide quality nutrition care; participate in weekly care plan reviews; support continuous quality of life for residents in a 200-bed rehabilitation and skilled nursing facility.
Required Qualifications
- Must be a Registered Dietitian
- Degree in Nutrition or Dietetics
- Exceptional interpersonal & leadership skills
- Knowledge of applicable state and federal guidelines
- Computer literacy and proficiency with EMR software and computer-based menu systems
