Job Summary
Conduct comprehensive nutrition assessments and follow-up evaluations for PACE participants, developing individualized care plans based on medical conditions and personal goals. Provide nutrition counseling and education to participants, caregivers, and family members while collaborating with physicians, nurses, and social workers to optimize outcomes. Monitor progress and modify interventions as needed, ensuring timely documentation in electronic medical records and active participation in interdisciplinary team meetings. Complete one on-site visit weekly with approximately 50% of responsibilities performed remotely. Support healthcare organizations across the United States by delivering tailored nutrition services to diverse client needs.
Required Qualifications
- Bachelor's or Master's degree in Dietetics, Nutrition, or a related accredited field
- Registered Dietitian (RD/RDN) credential through the Commission on Dietetic Registration (CDR)
- Minimum of one year of clinical nutrition experience preferred
- Experience working with geriatric populations or PACE programs is preferred
- Strong communication, organization, and time management skills
- Proficiency with electronic medical records and documentation systems
- Ability to work independently while collaborating effectively with an interdisciplinary team
