Job Summary
Conduct comprehensive nutritional assessments for new admissions and quarterly reviews, developing and monitoring individualized nutrition care plans. Provide nutrition education, monitor resident weight trends weekly or monthly, and perform monthly kitchen sanitation audits with corrective action recommendations. Collaborate with physicians, nurses, and therapists to ensure compliance with CMS and DOH regulations while maintaining appropriate documentation and reviewing menus semi-annually. Participate in quality improvement initiatives to enhance resident outcomes. This part-time role offers 16-24 hours per week with 50% remote telehealth flexibility, compensating at $40.00-$45.00 per hour.
Required Qualifications
- Bachelor's or Master's degree in Dietetics, Nutrition, or a related field from an accredited program
- Registered by the Commission on Dietetic Registration
- Current state licensure/certification (if mandated by the state)
- Minimum of 1 year clinical experience (preferred)
- Excellent communication skills for effective interaction with patients, families, and healthcare teams
- Possesses the ability to apply critical thinking to a variety of situations, assess objectives, develop and implement and monitor effective plans for improvement
- Proficiency in providing general nutrition interventions
- Familiarity with state survey processes and adhering to nutrition care regulations
- Competency in electronic charting systems
